Community social worker - Unhoused community needs assessment

Reports To: Pastor
Status: Part-time
Location: Trinity United Methodist Church Atlanta

Pay: $40 an hour, 10 hours a week

Position Summary

The Community Social Worker will support the church’s outreach mission by assessing available community infrastructures. This includes but not limited to the mental health and social service needs of unhoused individuals in hopes of strengthening and streamlining church outreach. This role bridges compassion, clinical insight, and community partnerships to create effective and sustainable support pathways.

Primary Objectives

  1. Assess needs of unhoused individuals and families

  2. Assess community mental health and social service resources

  3. Evaluate and strengthen Trinity UMC’s current outreach programs

Key Responsibilities

  • Conduct trauma-informed assessments with unhoused individuals to identify mental health, housing, medical, or social service needs

  • Build trust-based relationships with community members through outreach and on-site engagement

  • Map and maintain an updated directory of community resources, shelters, clinics, and service providers

  • Evaluate Trinity UMC’s current church outreach programs for effectiveness, accessibility, and impact

  • Develop referral pathways and care coordination systems

  • Collaborate with church leadership, volunteers, local church programs and external agencies

  • Track outcomes and prepare reports for leadership and funders

Qualifications

  • Degree in Social Work, Counseling, Psychology, or related field preferred

  • Licensed or license-eligible preferred (LCSW, LMSW, or equivalent)

  • Experience working with unhoused populations and mental health services

  • Strong assessment, case management, and collaboration skills

  • Ability to work compassionately in faith-based, community settings

Working Plan: Integrating Needs Assessment, Resources & Church Programs

Phase 1: Community Needs Assessment (Month 1–4)

Goal: Understand who the unhoused populations are and what they need most.

Actions:

  • Conduct intake interviews

  • Observe outreach services and community engagement patterns

  • Identify gaps in housing access, behavioral health care, employment, and benefits

  • Compile a needs assessment report

Deliverables:

  • Needs summary dashboard

  • Priority needs list

  • Risk and vulnerability indicators

Phase 2: Community Resource Mapping (Month 5-8)

Goal: Identify what services already exist and how to access them.

Actions:

  • Build relationships with shelters, clinics, hospitals, nonprofits, housing authorities

  • Create a referral directory with eligibility criteria and contacts

  • Identify service gaps and duplication

  • Develop warm handoff referral pathways

Deliverables:

  • Resource directory

  • Partnership agreements

  • Referral flowcharts

Phase 3: Church Program Evaluation (Month 9-12 )

Goal: Strengthen existing Trinity UMC programs to better meet real needs.

Actions:

  • Review current outreach, food programs, shelter partnerships, and volunteer models

  • Gather feedback from staff, volunteers, and service participants

  • Identify barriers, inefficiencies, and opportunities for expansion

  • Align programs with trauma-informed and mental health best practices

Deliverables:

  • Program assessment report

  • Improvement recommendations

  • Service redesign framework
